One is a permanent map. The other is today's weather. They get conflated constantly.
The words get used interchangeably and they mean different things, which is most of why astrology sounds incoherent to people outside it.
A natal chart
A snapshot of the sky at the exact moment and place you were born. It never changes. Everything permanent in astrology, character, structure, capacity, is read here.
A horoscope
In modern usage, a forecast. The daily kind sorts the world into twelve groups by Sun sign, which means it is describing a transit against one placement and ignoring the other forty. That is not a failure of astrology so much as a limitation of the format.
What a real forecast requires
Your actual chart, and the actual sky today, compared. Saturn crossing your Moon is a specific event with a date; Saturn crossing the Moon of everyone born in a four-week window is not an event at all.
The word itself
Confusingly, horoscope originally just meant the chart, from the Greek for observing the hour. Traditional astrologers still use it that way. If someone asks to see your horoscope and means your birth chart, they are the ones using it correctly.
